Our Continuum of Care ensures that our patients remain with their family at Family Health West as their health care needs change The patient feels secure in knowing that their needs can be met without the undue stress and anxiety of moving to an unfamiliar place We have onsite nursing and social work assessments to assure appropriate efficient placement and treatment Coordinated physical rehabilitative programs allow the patient to recover as quickly as possible close to home


History

Family Health West located in Fruita Colorado has remained on the leading edge of rural health care service delivery for over 60 years The 20bed hospital was a result of a community effort in the mid 1940s to provide health care for residents of rural Mesa County in Western Colorado Local farmers and ranchers drove to Utah and brought back loads of bricks and supplies to build the hospital

Although the hospital weathered many crises in the early years it survived because of the communitys commitment and dedication to maintaining local health care

In the 1980s Family Health West began to implement a broader range of health care services to meet the needs of the community The Continuum of Health Care was developed to include senior elder and Alzheimers care services Family Health West is now recognized as a pioneer and leader in health care services for individuals of all ages and in the implementation of innovative Alzheimers and eldercare

In 1997 Family Health West unveiled their quotMain Street Experiencequot a social model for Alzheimers and elder care This approach is designed to positively stimulate the longterm memories of Alzheimers patients in a nonthreatening enjoyable environment

Today
Family Health West has over 350 employees serving over 245 patients As a nonprofit provider 80 of the clientele are predominately lowincome Medicaid or Medicare patients Being the areas premier provider of Alzheimers and elder care since the mid1980s the goal is to continue to develop and provide stateoftheart programs for our patients resulting in a higher quality of life
