Toronto Grace Health Centre TGHC is a 119 bed facility owned and operated by The Salvation Army located in the heart of downtown Toronto Toronto Grace is renowned for the compassion clinical excellence and patient centered care we provide The heart of patientcentred care is recognizing that each person has individualized priorities and goals and each care plan reflects this Through understanding and appreciating the complexity of daytoday life Toronto Grace provides patientcentered care for individuals with multifaceted chronic diseases who require Complex Continuing Care Slow Paced Rehabilitation and Palliative Care

Our Complex Continuing Care CCC program serves persons with complex and multiple chronic diseases and functional disabilities who require a wide range of therapeutic services and clinical specialties The CCC program provides medical management skilled registered nursing care as well as therapy recreation and spiritual services The program offers care of the individual following a restorative and preventative approach The Grace offers comprehensive services to support individuals so they may enjoy their community family and friends

SlowPaced Rehabilitation provides care to persons who require a longer period in rehabilitation to make the most of their recovery Our dedicated team of rehabilitation experts includes Physiotherapists Occupational Therapists Nurses and Speech Language Pathologist Therapy treatment rooms are located on each unit to support an integrated therapy plan and rehabilitation Persons accepted into slowpaced rehabilitation look forward to returning home andor back into the community

Palliative Care provides comprehensive endoflife care to support the person and family at their end of life journey Our holistic approach to care includes a skilled interdisciplinary team to meet the individuals physical psychosocial spiritual and cultural needs Persons in the community who wish to remain at home for as long as possible can be rapidly be admitted into the Palliative Care Unit if needed

History

Toronto Grace Health Centre is a 119 bed palliative and complex continuing care facility that has been providing compassionate care in the heart of downtown Toronto for the past 100 years From its earliest days as a rescue home to its conversion to a maternity hospital and laterally to its current role of providing care to come of the citys most vulnerable the facility continues to provide care where the needs are greatest

The following is a brief history
1889 The Salvation Army opens a Rescue Home in Toronto


1905 The Home becomes The Salvation Army Maternity Hospital on Esther Street


1909 Hospital moves to the corner of Bloor and Church streets


1925 An extension is added to what was then called the Toronto Womens Hospital


1937 The hospital is renamed Toronto Grace


1959 New building is constructed at Bloor and Church


1969 Designated a general hospital


1979 Designated a chronic care and palliative care hospital opening the first Palliative Care Unit in the Province of Ontario


1998 The Health Services Restructuring Committee directed the Toronto Grace and three other complex continuing care hospitals to close


2001 The Ministry of Health reverses the decision and directs Toronto Grace to continue to provide its programs and services The Hospital Board begins to look at emerging needs in the health system


2005 Toronto Grace Health Centre celebrates its 100th birthday at Church and Bloor Street in downtown Toronto where it continues to provide its special blend of care

2007 Toronto Grace Health Centre launches its new logo and new look that reflects the care that is given to patients daily Toronto Grace shows itself to be a leader on providing SlowPaced Rehabilitation Palliative and Complex Continuing Care
